Dr. Raul Vazquez is a primary care physician who opened Urban Family Practice on Buffalo’s Lower West Side in 1996. In 2016, the National Committee for Quality Assurance (NCQA) announced that Urban Family Practice of Buffalo had received level three NCQA Patient-Centered Medical Home (PCMH). Level three is the highest degree of recognition available.

Recognizing the need for coordinated, person-centered care for Medicaid recipients with multiple chronic medical conditions, he formed the Greater Buffalo United Accountable Healthcare Network (GBUAHN). The New York State Department of Health approved the designation of GBUAHN as a provider-lead Medicaid health home in 2012.

As president of GBUAHN, Dr. Vazquez leads a team of dedicated professionals in the medical and business fields. He has set the strategy and direction for the fast-track growth of GBUAHN. Under his guidance, GBUAHN has grown to serve several thousand members and employs a staff of approximately 150 individuals. GBUAHN is one of the largest minority employers in Western New York.

In June 2009, Dr. Vazquez was nominated by Governor David Paterson, and approved by the New York State Senate, to serve as a member of the New York State Medicaid Advisory Committee. In June 2012, Governor Andrew Cuomo and the New York S Senate approved Dr. Vazquez to serve on the Minority Health Council.

He has won more than 20 awards for his accomplishments in business, the medical field and advocacy groups. Dr. Vazquez has served boards for numerous organizations, including: Upstate New York Transplant Services, HEALTHeLINK and WNED-TV.

Dr. Vazquez received his undergraduate degree from Fordham University and received his medical degree from the State University of New York at Buffalo.

Toni Vazquez is the chief of systems for GBUACO, co-owner of Urban Family Practice, co-founder of the Raul and Toni Vazquez Foundation. Mrs. Vazquez is a philanthropist and inspirational leader for aspiring business professionals. She is the recipient of several prestigious awards, including: 2016 Business First Women of Influence award; the 2016 25 Influential Black Women in Business award from Black Western New York Magazine (BWNY Magazine) and (along with her husband, Dr. Raul Vazquez), the 2015 Buffalo Urban League Family Life Award.

Chief Medical Officer (GBUAHN)

Dr. Fox is chief medical officer (CMO) for the Greater Buffalo United Accountable Health Network (GBUAHN). Dr. Fox started as a solo rural family physician in a small village near Canandaigua, New York. After practicing there for 12 years, he went into academic medicine in 1993 as associate program director for the Deaconess Family Medicine program, run in conjunction with Buffalo General Hospital.

After successfully rebuilding the program from eight to 20 residents, and increasing the faculty complement from two full-time faculty to six full-time faculty, he began a career in primary care research. His area of interest is improving care at the doctor’s office level and translating evidence-based medicine into practice utilizing practice-based research networks. He is considered an expert in the field of practice-based research.

Dr. Fox is recognized in both the United States and Canada for his expertise in chronic kidney disease. He is a Fellow of the National Kidney Foundation (NKF) and became the first primary care physician ever to serve on the national board of the NKF.

Dr. Fox was a principal investigator of a $3.7 million National Institute of Health study using computer decision support to improve the early recognition and treatment of chronic kidney disease. In 2015, his research work earned him a university award in recognition of outstanding achievement in scholarly activity.

In 2001, Dr. Fox founded the Lighthouse, a free clinic run by medical students. He also founded the Forensic Medical Unit for the Family Justice Center, a one-stop shop for domestic violence victims. In 2003, Dr. Fox became a charter member of the Richard Sarkin Chapter of the Gold Humanism Society.

Dr. Jesslyn Perry has been a physician since 1995. After working at Sheehan Hospital as an internist from 1999 -2001, she opened Ella Perry, M.D., located at 191 North Street in Buffalo, N.Y. She named her practice after her mother.

Dr. Perry specializes in family and internal medicine. She is affiliated with Women and Children’s Hospital of Buffalo and Buffalo General Hospital. She is board certified in Internal medicine. Dr. Perry is a member of the National Medical Association (NMA).

Dr. Perry is a graduate of the State University of New York at Buffalo School of Medicine and Biomedical Sciences.

Daniel Fells grew up in Great Neck, New York. He is a board-certified Physician’s Assistant, who began work as a PA at Urban Family Practice in 2004 after earning a Bachelor of Science Degree in Physician Assistant Studies from D’Youville College (Magna Cum Laude). Prior to completing his medical studies, Daniel was employed in the mental health field and as a Nurse’s Aid for in-home patient care. He prides himself in providing excellent patient care and believes in, “starting the way you want to finish.”

Casey Feeney is a board certified physician’s assistant who was born and raised in Buffalo, N.Y. She graduated from D’Youville College in 2017 with a Master’s of Science in physician assistant studies. She also earned a Bachelor of Arts in psychology from D’Youville College in 2009; and graduated from University at Buffalo with a Master’s of Arts in behavioral neuroscience in 2012.

Feeney began rotating at Urban Family Practice as a student under the supervision of Dr. Raul Vazquez and Daniel Fells, PA-C. During that time, she developed a great appreciation and love for the practice of family medicine in an urban setting.

Feeney began practicing at Urban Family Practice as a provider in February 2018. She hopes to continue to grow as a provider at Urban Family Practice.

Brendan Farnand, PABrendan Farnand, PA
Physician Assistant (UFP)

Brendan Farnand grew up in Rochester and moved to Buffalo to attend university in 2013. He received his BS/MS in physician assistant studies in 2017 from D’Youville College. Before graduation, he completed three clinical rotations with Daniel Fells at Urban Family Practice starting in early 2017. He also speaks fluent Spanish, after living for a time in Colombia.

Nurse Practitioner

Alison Greenberg is a board certified Family Nurse Practitioner from the Los Angeles area. She earned her Master of Science at Georgetown University in 2017. She worked as a critical care nurse at Roswell Park Cancer Institute in Buffalo, NY after receiving her Bachelor of Science in Nursing at New York University in 2011. She also holds a degree from University of California at Berkeley, where she studied fine art. She enjoys gardening, painting and playing the cello.

She joined Urban Family Practice in February of 2019, and appreciates working holistically with patients of all ages and helping them take an active role in their health.

Office Manager

Terrance Gaiter has been working in medicine for the past 20 years and has spent most of his career with Urban Family Practice.

Gaiter began his career as an office phlebotomist for a family practice. By 2007, he was firmly established at Urban Family Practice, as a medical assistant and trainer for support staff. In 2015 he was promoted to office coordinator. Gaiter continues to progress at Urban Family Practice, now running two locations as office manager.
