Our Mission
Incorporated in mid-2008, the Raul and Toni Vazquez Foundation’s sole purpose is to protect and enhance the abilities of nonprofit health care organizations to serve society and their individual communities.
Through research, public education, and advocacy, the foundation seeks to provide a strong, cohesive and persistent “voice” for this nonprofit health care organizations common goals and challenges with providers, and others.
In addition, through education and other types of programs, the foundation seeks to enhance the performance of nonprofit health care organizations in carrying out their unique roles and responsibilities.
Raul Vazquez, M.D. F.A.A.F.P.
Dr. Raul Vazquez is a primary care physician who opened Urban Family Practice on Buffalo’s Lower West Side in 1996. In 2016, the National Committee for Quality Assurance (NCQA) announced that Urban Family Practice of Buffalo had received level three NCQA Patient-Centered Medical Home (PCMH). Level three is the highest degree of recognition available.
Recognizing the need for coordinated, person-centered care for Medicaid recipients with multiple chronic medical conditions, he formed the Greater Buffalo United Accountable Healthcare Network (GBUAHN). The New York State Department of Health approved the designation of GBUAHN as a provider-lead Medicaid health home in 2012.
As president of GBUAHN, Dr. Vazquez leads a team of dedicated professionals in the medical and business fields. He has set the strategy and direction for the fast-track growth of GBUAHN. Under his guidance, GBUAHN has grown to serve several thousand members and employs a staff of approximately 150 individuals. GBUAHN is one of the largest minority employers in Western New York.
In June 2009, Dr. Vazquez was nominated by Governor David Paterson, and approved by the New York State Senate, to serve as a member of the New York State Medicaid Advisory Committee. In June 2012, Governor Andrew Cuomo and the New York S Senate approved Dr. Vazquez to serve on the Minority Health Council.
He has won more than 20 awards for his accomplishments in business, the medical field and advocacy groups. Dr. Vazquez has served boards for numerous organizations, including: Upstate New York Transplant Services, HEALTHeLINK and WNED-TV. Dr. Vazquez received his undergraduate degree from Fordham University and received his medical degree from the State University of New York at Buffalo.
Toni Vazquez
Toni Vazquez is the chief of systems for GBUACO, co-owner of Urban Family Practice, co-founder of the Raul and Toni Vazquez Foundation, and the current president of the Williamsville Schools Board of Directors. Mrs. Vazquez is a philanthropist and inspirational leader for aspiring business professionals. She is the recipient of several prestigious awards, including: 2016 Business First Women of Influence award; the 2016 25 Influential Black Women in Business award from Black Western New York Magazine (BWNY Magazine) and (along with her husband, Dr. Raul Vazquez), the 2015 Buffalo Urban League Family Life Award.
